Understanding Horizon BCBSNJ and ABA Coverage

Horizon Blue Cross Blue Shield of New Jersey is one of the largest insurers in the state. If you have a Horizon plan and your child has an autism diagnosis, you are likely eligible for applied behavior analysis (ABA) therapy. However, coverage details vary by plan, so you must verify with Horizon directly. Do not assume that ABA is covered just because you have insurance. Call the customer service number on the back of your insurance card and ask specifically: "What is my ABA benefit? Is there a deductible, copay, or coinsurance? Do I need a referral or prior authorization?" Write down the name of the representative you speak with and the date.

What Horizon BCBSNJ covers for ABA

New Jersey law requires many health plans to cover ABA for children with autism. But Horizon plans differ. Some are employer-sponsored, some are individual plans, some are through NJ FamilyCare (Medicaid/CHIP). If your child is on NJ FamilyCare, your managed care plan may be Horizon NJ Health, Wellpoint, Aetna Better Health of NJ, or UnitedHealthcare Community Plan. Horizon NJ Health is a separate entity from commercial Horizon BCBSNJ, so the rules may be different. For children under 3, the New Jersey Early Intervention System handles services, not insurance. For ages 3 through 21, you access the Children's System of Care through PerformCare. PerformCare is the single access point. You can call PerformCare to start the process. They will help you get an evaluation and connect you to providers, including those who accept Horizon.

Finding an ABA provider in Union County

Union County has 52 ABA providers registered on the federal NPPES registry. That is a good starting point. Every one of the 21 municipalities in Union County has at least one provider within 7 miles, so you do not need to travel far. The towns with the most providers are Union (9), Cranford (7), Clark (6), Westfield (6), Elizabeth (4), and Linden (4). If you live in a smaller town like Garwood or Mountainside, your closest provider may be in a neighboring town. To find providers that accept Horizon, you have a few options:

  • Search the NPPES registry online (look for providers with a taxonomy code for ABA).
  • Call Horizon's provider directory or log into your online account.
  • Ask your child's pediatrician or school district for recommendations.
  • Contact PerformCare (for children 3 and older) or the Early Intervention System (under 3) for a list of approved providers.

When you call a provider, ask: "Do you accept Horizon BCBSNJ? Do you have openings? What is the process for getting started?" Be prepared for a waiting list. We cannot tell you exact wait times because they change constantly. Some providers may have immediate openings, others may take months. Keep calling.

Steps to get started with Horizon BCBSNJ

  1. Get a diagnosis. ABA therapy requires a formal diagnosis of autism spectrum disorder. Your pediatrician, a developmental pediatrician, or a child psychologist can provide this. If you need help finding a diagnostician, contact PerformCare or Early Intervention.
  2. Verify your coverage. Call Horizon and ask for your ABA benefit. Ask if you need a referral from your primary care doctor. Some plans require it. Ask about prior authorization: many plans require the provider to submit a treatment plan before sessions start.
  3. Choose a provider. Use the methods above to find a provider that accepts Horizon and has availability. You may need to try several.
  4. Complete intake. The provider will ask for your insurance card, diagnosis, and possibly a prescription for ABA. They will handle the prior authorization with Horizon.
  5. Start therapy. Once authorized, sessions can begin. Keep records of all communications with Horizon and the provider.

Working with your child's school district

Your child may also receive ABA-related services through their school district if they have an Individualized Education Program (IEP). Union County has many public school districts, including Berkeley Heights School District, Clark Township Public School District, Cranford Public School District, Elizabeth Public Schools, Garwood Boro, Hillside Public School District, Kenilworth School District, Linden Public School District, Mountainside School District, and Plainfield Public School District. If your child is in one of these districts, request an evaluation for special education. The district may provide ABA as a related service. However, school-based ABA is separate from insurance-based ABA. You can use both, but the school cannot bill your insurance. Talk to the child study team in your district. Ask: "Can ABA be written into my child's IEP?" The answer depends on your child's needs and the district's resources.

When your child turns 21

After age 21, the Children's System of Care ends. At that point, the Division of Developmental Disabilities (DDD) becomes the main agency for adults with autism. DDD has its own eligibility process and funding. If your child is on Horizon BCBSNJ, they may still have insurance coverage, but the rules may change. Plan ahead. Contact DDD at least a year before your child turns 21 to start the transition.

Common questions

Does Horizon cover ABA for adults?

It depends on your plan. Some Horizon plans cover ABA for adults, but many do not. You must check with Horizon directly. If your child is over 21 and on NJ FamilyCare, the Medicaid plan may cover ABA, but again, verify.

What if my provider says they don't accept Horizon?

You can ask the provider to apply for an in-network exception or single-case agreement with Horizon. This is not guaranteed. You can also file an appeal with Horizon if you believe the provider is the only one available within a reasonable distance. In Union County, with 52 providers, you may find another that accepts your plan.

Can I use both Horizon and NJ Early Intervention?

For children under 3, Early Intervention is the primary system. They may coordinate with your insurance, but you do not need to use Horizon for services under 3. After age 3, PerformCare takes over, and insurance often plays a larger role.
